CSV field guide

Empty amount cells mean no amount is recorded, not a zero charge. Monthly, annual, and unknown-period required memberships have separate columns. Use the disclosure fields to distinguish an unpublished fee from one that has not been checked.

entry_id

Permanent program identifier, never reused.

status

active, withdrawn, or not-verified. This is the recorded program status, not clinical approval.

provider

Company name.

provider_domain

Recorded provider website domain.

drug

Recorded medication category. Unqualified semaglutide, tirzepatide, liraglutide and dulaglutide identify the ingredient without establishing brand, generic or compounded formulation. Brand exenatide records a brand category without establishing dosage form or indication. Generic exenatide records a generic category without establishing a selected manufacturer, device, approval or availability. A combined category records one program containing two identified ingredients, without establishing a single coformulated dosage unit. See the program record for qualifications.

form

Recorded delivery form. injection-unspecified establishes injection but not the device; oral-unspecified establishes oral medicine but not the precise dosage form or administration instructions. not-disclosed means the form has not been established in the record.

lead_price_usd

Selected comparable calendar-month figure or equivalent. Explicit introductory, conditional, nonmonthly and unverified payments are excluded. Read the plan label; this is not necessarily a monthly bill or total cost.

lead_price_label

The selected price's plan, dose and other recorded qualifications.

lead_price_term_months

Medication commitment stored with that price. Check the original record for billing-period limitations.

cheapest_term_usd

Lowest comparable calendar-month figure or equivalent, potentially requiring a longer commitment. Uses the same exclusions as lead_price_usd.

cheapest_term_label

Qualifications attached to the lowest price-ladder figure.

required_membership_usd

Recorded additional recurring monthly membership amount, only when required and not included in the program price. It may differ from an introductory fee.

required_membership_amount_no_period_usd

Additional required membership amount whose billing period is unstated or unresolved. Never assume it is monthly.

consult_fee_usd

Recorded consultation amount, or the lower endpoint when a range is published. See JSON or the full record for the upper endpoint and conditions.

consult_fee_disclosure

disclosed, disclosed-not-priced, none, not-disclosed (checked), or not-checked.

pharmacy

Recorded pharmacy or network text. A named network does not establish which pharmacy will fill an individual prescription.

pharmacy_disclosure

disclosed, not-disclosed (checked), or not-checked.

first_recorded

Exact ISO date when the program was first recorded.

last_verified

Exact ISO date of the program's latest verification. Individual sources may be older.

observations

Number of historical readings retained for this program.

evidence_url

For entries without a comparable monthly rung, this is the first published price source. Source URL for the cheapest price-ladder figure, which may differ from the selected lead price.

required_membership_annual_usd

Additional required membership charged once a year. Kept separate from monthly and unknown-period fees.

membership_disclosure

disclosed, disclosed-not-priced, none (explicitly no membership), not-disclosed (checked), or not-checked.

membership_required

true when recorded as required; false when optional or explicitly absent; blank when conditional or when the requirement is unknown.

consult_fee_included_in_program

True means the recorded consultation amount is already included in the program price and must not be added again. Blank means no separately structured fee record.

membership_condition

Eligibility or payment-path condition. Conditional fees are excluded from universal required-membership amount columns and membership_required is blank.

membership_period_conflict

True when published billing periods conflict. The recorded amount must not be assumed monthly.

membership_included_in_program

True means required provider care is already included in the program price. Do not add it again; included amounts are excluded from required-membership amount columns.

membership_period_amount_usd

Recorded day/week care fee. Read membership_required, membership_condition and membership_included_in_program before treating it as an additional required charge; never sum as monthly.

membership_period_unit

Exact recurring care-fee unit: day or week. No calendar-month conversion.

membership_period_count

Number of days or weeks per recorded care-fee period.

membership_period_evidence_url

Provider source directly evidencing the day/week care-fee amount and period. Dated quotation remains in the JSON record.

ingredient_families

Semicolon-separated identified ingredients in this one program; not separate invoices or proof of coformulation. Empty means ingredient identity is not established, not that no active ingredient exists.
