FAQ explicitly states medication, prescription, consultation and shipping are billed as ONE monthly fee: "The cost of medication, prescription, clinical consultation and shipping are all included in one monthly fee," and separately: "Please note, lab costs aren’t covered in your membership fee" — Levity does use the word "membership fee" for its all-in bundle, but since that fee includes the medication (only labs are excluded), it does not meet this register's definition of a membership-that-excludes-the-drug, so it was not recorded under `membership`.
