Recorded exactly as printed (49 states, no editorializing to '50' and no silent addition of the missing one). NuBloom explicitly states it deliberately does NOT prescribe GLP-1 in pill form as a clinical choice ('At NuBloom, GLP-1 treatment is offered as an injection rather than a pill... that is why we only offer GLP-1 injections'), so form is very likely a vial or pen, but the word 'vial' is never used, so per the brief's caution against inferring, form is left not-disclosed.
